Each July, the medical community highlights one of the most misunderstood cancers. Despite affecting thousands annually, nearly nine out of ten people cannot identify it.
Symptoms are commonly attributed to benign issues like muscle or soft tissue injury. This knowledge gap can prevent timely detection and have a significant impact on treatment outcomes. When healthcare professionals fail to recognise symptoms, misinterpret imaging or other investigations, or fail to put in place an appropriate treatment and surveillance plan, the consequences can be devastating.
This is the first article in a two-part series on sarcoma. In this article, we focus on what sarcoma is, why awareness matters, and the common ways it presents. In part two, we explore the diagnostic journey, treatment approaches, and the legal issues that can arise when diagnosis is delayed.
Understanding sarcoma and the need for greater awareness
Sarcoma is a type of malignant tumour which develops in connective tissues, including bones, muscles, and blood vessels. With over seventy subtypes, these rare cancers account for approximately one per cent of adult cancers, although they account for a higher proportion of cancers affecting children. Their rarity means general practitioners may encounter only few cases throughout their career, increasing the risk of misdiagnosis as sports injuries or benign cysts. Sonographers and radiologists may also have limited experience of their presentation on imaging. Delayed diagnosis significantly impacts treatment options and survival prospects, making awareness campaigns vital.
Diagnostic delays create significant psychological distress beyond physical implications. Patients often feel dismissed when reporting persistent symptoms, causing frustration and isolation. When a diagnosis is finally confirmed after a prolonged period of uncertainty, it can bring a complex mix of emotions, including relief at having an explanation for their symptoms and anger or regret over lost time and missed opportunities for earlier intervention. This highlights the importance of heightened awareness and vigilance among healthcare professionals.
Common subtypes of sarcoma and how they present
Soft tissue sarcomas include numerous subtypes with distinct characteristics. For example, liposarcoma develops in fatty tissue, typically in thighs or retroperitoneum, whilst synovial sarcoma develops near joints in the extremities, predominantly affecting younger adults.
Bone sarcomas present unique challenges. Osteosarcoma typically affects adolescents around the knee joint, and Ewing sarcoma frequently affects the pelvis or femur in children, teens and young adults, whilst chondrosarcoma predominantly affects adults over forty in the pelvis or shoulder.
Sarcoma presentation varies by location, size, and subtype. Most present as painless lumps gradually increasing over a period of weeks. Deep-seated tumours may grow substantially before detection, whilst those near nerves can cause pain or numbness early.
Recognising warning signs and symptoms of sarcoma
Where key warning signs are present, investigation is vital:
- any lump persisting beyond four weeks, particularly if enlarging, warrants assessment.
- lumps exceeding five centimetres or located deep beneath fascia require urgent specialist referral.
- persistent pain should not be dismissed as a symptom of a more common and less serious condition. Malignant tumours can cause pain through nerve compression or bone infiltration, and pain overnight which may wake a patient deserves thorough investigation, especially in younger patients.
- functional impairment such as reduced joint mobility, limb weakness, or difficulty with routine activities is a key diagnostic sign. Tumours near major nerves can cause numbness, tingling, or radiating pain. Gastrointestinal sarcomas may present with swallowing difficulties, nausea, weight loss, or altered bowel habits. Retroperitoneal tumours can cause abdominal discomfort or early satiety, often leading to misdiagnosis as gastritis or irritable bowel syndrome.
- constitutional symptoms like unexplained weight loss, fatigue, night sweats, or fever may indicate advanced disease. In children, reduced activity participation or persistent discomfort warrants investigation.
Persistent, progressive symptoms require thorough evaluation rather than being attributed to more common benign causes.
